Education Book a course Six Steps to Success End of Life Care Programme The Six Steps to Success Programme is led and delivered by the Mountbatten Education Team at Mountbatten Hampshire (formerly Countess Mountbatten). The programme includes training days at our Education Centre alongside virtual support sessions provided by a designated facilitator assigned to each home. The aim is to improve end of life care through facilitating organisational change and supporting staff to develop their roles around end of life care. The six steps programme covers all aspects of end of life care from early discussions through to bereavement care. Benefits: Good opportunities for networking with peers Consolidation of current skills and knowledge and acquisition of new skills and knowledge Improving ability and confidence with end of life care; identifying best practice Improved satisfaction of families and patients and job satisfaction to staff Greater awareness of national and local strategies/policies as drivers for end of life care Help in developing better multidisciplinary team working Opportunities for all staff to attend teaching sessions on palliative care Improving outcomes in end of life care such as reduced hospital admissions, patients dying in place of choice etc. Programme: The programme consists of 1 half study day and 3 full study days encompassing all aspects of the six steps pathway. This will include the six steps and a conclusion. There is an expectation that a portfolio of evidence will be completed at the end of the programme to demonstrate how the care home has implemented the six steps into practice. The care homes must be committed and engaged to this process by supporting 2-3 staff members to attend the full programme. With one staff member being a senior member of staff who can support, embed and sustain organisational change within the care home.
