The team's work includes helping with pain and symptom control, end-of-life planning, giving practical advice and supporting patients and their families.

Where appropriate, Mountbatten's expert nurses and doctors may visit the homes of people dealing with death and dying, in Southampton and across large parts of Hampshire.

Care at home team

Depending on your circumstances, you may be eligible for a package of care delivered by Mountbatten Hampshire's care at home team.

This may include regular home visits to support you with personal care needs. 

How can I access Mountbatten Hampshire's care at home team?

An application must be completed by a healthcare professional, such your district nurse, GP, social worker or hospital ward manager.
