Aim:  To discuss the benefits and challenges of Advance Care Planning, how to create ACPs for patients and have ACP conversations with patients and their families

• To examine Advance Care Planning and the value to the people within our care
• To explore how and when we commence discussions and identify any potential barriers to creating an ACP
• To explore creative ways to start an ACP conversation
• Explain the legal framework which underpins ACP
• To consider the impact poor planning has on future care
