Introduction:

The new Mountbatten End Of life Programme is led and delivered by the Palliative Care Education Team at Mountbatten Hampshire. The programme includes training day sessions at Mountbatten Hampshire alongside support sessions held within the Care Homes / Organisations.

The aim is to provide education on all aspects of end-of-life care, improve skills and confidence and ensuring an organisational approach to the end-of-life care that is delivered. Focusing on quality and the sustainability of quality after the Programme.

The Mountbatten Programme covers all aspects of end of life care from early discussions through to bereavement care.

The Programme:

The programme consists of an introduction meeting and 4 full study days encompassing all aspects of the Mountbatten Programme for up to 3 End of Life Champions to attend training days, lead on Service Improvement Project and disseminate teaching to all staff. One of the Champions should be a senior member of staff who can support, embed and sustain organisational change within the Organisation. There is an expectation that the home manager or deputy will attend the first day training session.

In order to complete the Programme, your Organisation is required to identify an area of development (related to EOLC) and work towards implementing changes.
You will be required to present your project and write a reflective account of the process.
The project will need to capture to CQC KLOEs – Safe, Effective, Caring, Responsive, Well Led.
Organisations will be supported through the process by the Mountbatten Hampshire Education Team.

This is followed by a Project Presentation Afternoon whereby all attended Mountbatten Champions present how they have achieved their organisational change project and receive certification of completion.
